This service is designed to help children (under 18 years old) and their families in the ACT and surrounding regions coordinate health care from a variety of professionals.
About this service
PLaNS is a single point of contact for families to help them get the care, information and support for their child when and where they need it.
It is available for people who live in the ACT and surrounding region.
We can help someone aged under 18 years old who has complex health needs and:
- travels outside the ACT for specialist medical care, and/or
- sees multiple health professionals or services for their care.
We can:
- help you coordinate care for your child
- make sure you have a say in your child’s health care planning and decision making
- help you anticipate barriers and solve problems in your child’s health or care
- give you the day-to-day support to help with your child’s health – at home, at school, or in the community
- connect you with the health and community services you need.

Families with children under the age of 18.
How to access this service
You do not need a GP referral for this service.
Family members, carers, GPs and other health professionals can also refer you to this service.

What to expect
Our team members will meet with you to get to know your child and your family and discuss if PLaNS is the right service for your needs. This can include talking about your child’s health needs and any services that already help you. We will ask your permission to contact these services to organise your child’s care.
We will involve you and your child in developing a care plan. We can also help young people and their families with a plan for using adult services once they turn 18.
If your family moves interstate, we can help you find the services you need there.
