The Transitional Therapy and Care Program (TTCP) is a short-term, goal-oriented program that aims to improve the independence of older people after being in hospital.

About this service

The Transitional Therapy and Care Program is a 12 week program that can be provided in your home,  at the Mullangarrie Unit in Red Hill or a combination of both. Our team members will talk with you to decide the best place for your care to be delivered.

The Mullangarrie Unit is a 14-bed residential rehabilitation facility located in Red Hill. How long you stay at the Mullangarrie Unit will depend on your goals and progress after hospital. Our aim for your stay is usually to be a maximum of 3 to 4 weeks, with a majority of the program taking place from your home.

Depending on your goals and care needs, services may include:

  • working with a care facilitator on your goals and care
  • help with personal care
  • help with cleaning and shopping
  • transport to medical appointments
  • nursing support
  • low intensity therapy such as physiotherapy, speech pathology or occupational therapy
  • help with your diet

Who we care for

We care for people who:

  • Live in the ACT (NSW residents can access Transitional Care via their local provider or may be able to access the Mullangarrie Unit in some cases).
  • Are over 65 or over 50 if Aboriginal or Torres Strait Islander
  • Have therapy goals to improve physical, cognitive or psychosocial function. For example, “I want to be able to have a shower by myself”.
  • Are able to actively participate in therapy that will help achieve their goals.
  • Would benefit from low intensity therapy.

How to access this service

The Commonwealth Government provides guidelines and eligibility criteria to access the program.

To access this service you need to:

  • be staying in hospital and remain in hospital during this process
  • be medically stable and have an Aged Care Assessment (a liaison officer will coordinate this process and be available to discuss and answer any questions)

What to expect

We will meet with you in hospital to talk with you about the Transitional Therapy and Care Program.

After you are at home, we will call you to make a time to visit you. You will usually receive a call within the first week that you are home.

If you are in the Mullangarrie Unit you will be seen by an allied health professional within the first few days.

We will ask you what things you would like to achieve (your goals) during the program. We will also complete assessments that tell us about your current ability and will usually include looking at you walking or doing activities that you want to improve. We will then talk with you about any further appointments.

Part of your rehabilitation program will include a home exercise program that you can safely complete outside of our visits. The more of the program you can complete, the better your recovery is likely to be.

  • We will let you know if we need any medical letters or other information to help us to give you the best possible service.

Where we’re located

We provide mostly home-based therapy and may also include some appointments in other community locations such as the Mullungarrie Unit in Red Hill. This will depend on your needs and goals.

If you are staying at the Mullangarrie Unit, this is where your therapy will be based. We will always discuss your rehabilitation program with you throughout your time with us.
